Gravity Forms - Create a New Form

Step-by-Step:

1. Make sure both Gravity Forms and the UkuuPeople Gravity Forms Add-on are installed.

2. Create your form fields in Gravity Forms and save.

A. Forms > New Form 

B. Give your form a title and (optionally) description.

C. Select your fields.

D. Edit the fields by clicking on each field you want to edit. A box with options pops up as seen below.

E. Save/Update form.
